To make your first visit as smooth as possible, we’ve provided all the necessary forms and important information in one convenient location. Here, you can access everything you need to get started with your care, including registration forms, insurance details, and what to expect during your initial consultation.
New Patient Forms
If you would prefer, you may print out the new patient registration packet fill it out prior to your appointment and bring it with you.
Please bring the following forms and information to your appointment:

• Driver’s license or valid photo ID
• Any relevant medical records including MRI and CT films and/or CD’s as well as the corresponding reports
• Physician referral forms if required by your insurance plan
• List of your current medications including all vitamins and supplements

Please be aware that Florida Neurology does charge a no-show fee if you cancel an appointment with less than 48-hours advance notice. The fee for missing a scheduled office visit is $75 and the fee for any missed procedures is $100. Please take into consideration that missing a scheduled appointment trivializes the time of our doctors and staff––time which may have otherwise been spent treating another patient.
Along with our three office locations, our physicians and physician’s assistants also see patients at our three affiliated hospitals: Central Florida Regional Hospital in Sanford, Florida Hospital Fish Memorial in Orange City, and Florida Hospital Waterman in Tavares.
Medication
If you are having difficulty affording your medication(s), regardless of your circumstances, please consider the following resources. For many neurological and psychological conditions, brand-name medications may provide greater efficacy for certain patients. The resources below offer access to various medications, including brand-name drugs, at a reduced cost.
Please also inform your Florida Neurology provider about your situation, as they may be able to assist you in gaining better access to your prescription medication. Taking your medication as prescribed is essential for achieving the best possible outcomes in your treatment plan.
