Established Patients:

Please contact any of our offices to schedule an appointment. If it has been longer than a year since you last visited our offices, please fill out the new patient registration packet on the Forms and Links page and bring it with you to your appointment.

 

New Patients: 

Please arrive at least 20-30 minutes prior to your appointment time in order to complete your new patient paperwork. If you would prefer, you may print out the new patient registration packet on the Forms and Links page and fill it out prior to your appointment and bring it with you.

Please remember to bring the following forms and information to your appointment:

• New Patient Registration form (on Forms and Links page)

• Insurance card(s)

• Driver’s license or valid photo ID

• Any relevant medical records including MRI and CT films and/or CD’s as well as the corresponding reports

• Physician referral forms if required by your insurance plan

• List of your current medications including all vitamins and    supplements

This information is necessary in order to give our doctors and staff the full opportunity to adequately assess and treat you. Please call us if you have any questions. Our main goal is to ensure that we fully address all of your concerns in the time that we spend with you.

Please be aware that Florida Neurology does charge a no-show fee if you cancel an appointment with less than 48-hours advance notice. The fee for missing a scheduled office visit is $60 and the fee for any missed procedures is $80. Please take into consideration that missing a scheduled appointment trivializes the time of our doctors and staff––time which may have otherwise been spent treating another patient.

Along with our three office locations, our physicians and physician’s assistants also see patients at our three affiliated hospitals: Central Florida Regional Hospital in Sanford, Florida Hospital Fish Memorial in Orange City, and Florida Hospital Waterman in Tavares.
